Polycythemia vera (PV) is a rare blood cancer. It forces your body to overproduce red blood cells. There is no cure yet. But treatments exist. They work. They lower complications and ease symptoms.
Even with good care, flare-ups happen.
You might wonder why. Environmental shifts? Poor habits? Often, it’s both.
“Symptoms can worsen due to environmental… dehydration, lack of sleep, high emotional stress,” Mohamad Khasawneh MD says. He is a hematologist at Loyola Medicine.
You can’t stop every symptom. But you can cut back on them. Start by spotting the triggers.
Environmental Shifts
PV makes blood thick. Too thick. This slows circulation.
High altitudes make it worse. Thin air means less oxygen. If you live in the mountains or travel there, watch out. Your blood already struggles for oxygen. This adds pressure.
Temperature swings matter too. Extreme heat or cold. Because blood flow is sluggish, hands and feet suffer. They are vulnerable to injury.
Then there is the itch. Specifically aquagenic pruritus. Hot showers trigger it. Intense itching breaks out after skin hits warm water.
“Itching is a common symptom… A common trigger for this symptom is a hot,” says Jorge Cortes MD. He is deputy director at UAB’s O’Neal Cancer Center. Sometimes the itch comes before diagnosis.
What You Eat and Drink
Diet plays a role. Not just what you eat, but how it affects blood volume.
Alcohol. It is a diuretic. Drains water. Dehydration increases clot risk. PV already raises that risk. Combine them? Dangerous.
Caffeine. The story is mixed. Some research links regular coffee lower PV risk. But if you have PV? Caffeine acts as a diuretic. Hydration drops. The FDA suggests 400 milligrams daily. That’s roughly two or three 12-ounce cups. Don’t overdo it.
Red meat and purines. PV raises gout risk. Painful joints. Attacks triggered by specific foods. Red meat? Yes. Alcohol? Yes. Also shellfish. Anchovies. Mackerel. Sugary drinks too.
Lifestyle Factors That Aggravate PV
It’s not just what you ingest. It’s what you do. Or don’t do.
Smoking. It narrows blood vessels. Cardiovascular disease risk goes up. Blood flow gets harder. Clots become more likely.
Inactivity. Sitting still slows blood further. Clots loom larger. Fatigue worsens too. You need movement. Even light exercise helps flow.
Dehydration. Again, the villain. Thin blood flows. Thick blood clots. Dehydration thickens it. The risk spikes.
Skipping meds. Adherence is key. Management focuses on lowering red cell counts. Miss doses? Flares follow.
“Symptoms are mostly triggered by a higher red cell count… visual disturbances, burning pain… enlarged spleen,” Cortes explains.
Other Health Conditions
High blood pressure. High cholesterol. Diabetes.
These aren’t just separate issues. They stack. They increase heart disease risk. They raise the odds of clots, heart attacks, strokes.
Keep them in check. They complicate PV.
Track Your Triggers
Sometimes a flare makes sense. Sometimes it’s baffling.
Why today? Why now?
Logging helps. Track daily activities. Note symptoms. Identify patterns. It reveals routine changes that act as triggers.
“It is highly recommended to track… simple daily food and symptom journal… makes it easier to spot patterns,” Khasawneh advises. Apps work too. Or paper. Whatever sticks.

Triggers don’t always hit fast. Sometimes they lag.
Hours? Maybe days. A delayed reaction. This makes tracing the cause hard.
Use the three-day rule. Look back. Scan your log for the past 24 to 72 hours before a flare. Find the culprit.
“Using a ‘three-day rule’… trace delayed lifestyle or dietary triggers,” Khasawneh notes. It’s effective for those sudden spikes.
